Medical Disclaimer & Patient Information
Please review this information before participating in any services offered by THE WELL.
Our Commitment to Safe, Evidence-Informed Care
At THE WELL, we are committed to providing personalized, evidence-informed wellness and performance medicine under the supervision of licensed healthcare providers. Our goal is to help patients optimize their health through comprehensive evaluation, individualized treatment plans, and ongoing medical oversight.
The information provided on this website is intended for educational purposes only. It should not be interpreted as medical advice, diagnosis, or treatment, nor should it replace the advice of your primary care provider or other qualified healthcare professional.
Medical Services
All medical services offered by THE WELL — including telemedicine consultations, IV nutrient therapy, weight optimization, hormone optimization, peptide therapy, regenerative medicine, recovery therapies, and other wellness services — are provided only after evaluation by a licensed medical provider.
Treatment recommendations are individualized based on each patient's medical history, current medications, laboratory testing (when appropriate), physical examination or telemedicine assessment, and clinical judgment of the treating provider. Not every patient is an appropriate candidate for every therapy.
Off-Label and Compounded Medications
Many medications used in modern medical practice are prescribed "off-label." This is a common and legally accepted practice when, in the judgment of a licensed provider, it is appropriate based on clinical evidence and patient-specific factors.
Some treatments offered by THE WELL may include compounded medications prepared by state-licensed and federally regulated compounding pharmacies (503A or 503B facilities, as applicable). Compounded medications are customized for individual patients and are not reviewed or approved by the U.S. Food and Drug Administration (FDA) in the same manner as commercially manufactured medications.
Peptide and Regenerative Therapies
Certain peptide and regenerative therapies offered at THE WELL may be supported by emerging clinical research but may not be FDA-approved for the specific use for which they are prescribed.
These therapies are individualized medical treatments. Potential benefits, risks, and limitations are discussed during the informed consent process. Patients should understand that clinical evidence is still evolving, long-term outcomes may not be fully established, individual responses vary significantly, and no treatment guarantees specific results.
Risks of Treatment
All medical treatments carry potential risks. Depending on the therapy, possible risks may include injection site reactions, bruising, infection, allergic reactions, gastrointestinal symptoms, changes in blood pressure or blood sugar, laboratory abnormalities, medication side effects, and unexpected adverse events.
Your provider will review known risks, benefits, and alternatives prior to treatment. Patients should promptly report any concerning symptoms and seek emergency medical care when appropriate.
Patient Responsibilities
Safe and effective care requires active patient participation. Patients are responsible for providing complete and accurate medical history, disclosing all medications and supplements, reporting allergies and prior adverse reactions, completing recommended laboratory testing, following prescribed treatment instructions, and reporting new symptoms or health changes.
Failure to provide accurate information may increase health risks. All patients must complete informed consent prior to initiating therapy.
Relationship With Your Primary Care Provider
THE WELL is designed to complement — not replace — your primary care physician or specialist. We strongly encourage patients to maintain ongoing care with a primary provider for preventive health services, chronic disease management, vaccinations, and routine screenings.
THE WELL does not provide emergency medical services. If you are experiencing a medical emergency, call 911 or seek immediate care at the nearest emergency department.
Active-Duty Military Personnel & Professional Athletes
Certain medications, peptides, supplements, or performance-oriented therapies may not be permitted under Department of Defense, military branch regulations, or professional sport organizations.
Active-duty service members, reservists, National Guard members, and professional athletes should consult their healthcare provider or command structure prior to initiating therapy. Patients are responsible for ensuring compliance with applicable military and professional sport organization rules and regulations.
Results Disclaimer
Individual outcomes vary significantly. THE WELL does not guarantee results in any of the following areas: weight loss, athletic performance, recovery, energy, hormone levels, body composition, longevity, sexual wellness, or disease prevention.
Outcomes depend on multiple factors including medical history, lifestyle, nutrition, sleep, exercise, genetics, adherence, and individual biology.
Insurance & Payment
Unless otherwise specified, services at THE WELL are elective and are not billed to commercial insurance plans. Patients are responsible for understanding all out-of-pocket costs prior to receiving care, including consultations, laboratory testing, medications, procedures, and membership services.

Limitation of Liability
By choosing to receive care through THE WELL, patients acknowledge they have been informed of the potential risks, benefits, alternatives, and limitations associated with their care.
